We make it easy to file a life insurance claim
In most states, your beneficiary- the person you choose to receive the cash payment after your death- can call us to file a claim. Or call us to find out the status of a claim at 1-800-523-5065, Monday through Friday, 8 a.m. to 7 p.m. ET.
Your beneficiaries may also ask your employer to file the claim for them.
We will need:
- A certified copy of your death certificate
- Eligibility information and signed beneficiary records from your employer.
- Police or fire reports from the investigating agency if your death is caused by an accident.
- Claim form(s)
We usually pay life claims within 10 to 15 business days. Claims when there is an accident can take longer.
